The publication, adapted from a Victorian book by the same name, and produced by InterACT has been tailored for South Australian families through the support of a team of health professionals coordinated by Anna Malden, Chronic/Complex Care Discharge Clinical Practice Consultant, Patient Flow at the Women’s & Children’s Hospital.
